Scroll down until you see the grey boxes. Each box represents one work on your performance.
At the bottom of each page (after three grey boxes) are buttons that you click to make items to drag into the grey boxes. The buttons work only on the page they are located.
The red button resets the page. You should always click the red button before you start your form.
The green button creates a pdf of this form. Click it ONCE when you have finished filling the form out. Touch nothing until the PDF has finished downloading.
Attach the PDF to an email and send it to dmason@esm.rochester.edu.
