Eastman School of Music Instrument Locker Request

Lockers at the Eastman School of music are available to collegiate students to store instruments. They are available on a first-come, first-served basis. Expect a response to your request within 5 business days. Contact the Office of Student Life at studentlife@esm.rochester.edu or 585.274.1106 with any questions. **There are no lockers currently available. In order to be put on the waiting list, fill out the form below. We will e-mail you if a locker becomes available for you.**

Full Name (signed by a parent/guardian if under 18) * By requesting this locker and signing my name, I understand that this locker will be assigned to me for the 2012-2013 academic year (Aug 20, 2012- May 20, 2013). I will use the lock that is supplied by Eastman and communicate any issues with the Office of Student Life. The lock and locker are property of the University and the University reserves the right to enter at any time. The University assumes no responsibility for loss or damage to locker contents. Failure to abide by these policies or e-mail communications throughout the year may result in having locker privileges revoked.
