1. Download and print the Student Broadcast Clearance Form. Fill out the form and return it to the T&MP Office in ESM 60. You can drop it off to us, mail it to us (Box 154), or fax it to us at 585-274-1001.
2. Make sure the form is turned in before performing in any Eastman concerts or recitals.
