Excused Absences Policies - Missing Ensembles

Approval from Kathy Zager must be obtained by the student and presented to the Office of Academic Affairs by using this form before the request for absence can be processed. It is the student's responsibility to obtain and submit this documentation in a timely fashion. Failure to do so will result in a rejection of the student's request for an excused absence.
